Middle School Sports Athletic Physical Form and Athletic Packet: These forms are REQUIRED for all students who make the Middle Schol Athletic team. These forms are NOT required to try out for a Team. Click HERE to download the Athletic Packet which includes the Physical Form. All forms will need to be turned in to the Office.
Conversations and questions about PE:
Physical Education(PE) is required for all students by California State law.
Injury: If your child is injured, you can write a note and submit it to the health clerk in the office for a PE excuse. The PE excuse is valid for up to 3 days, after this time an ORIGINAL SIGNED Doctor's note is required.
Religious: If you would like your child exempt from certain activities or for a period of time in PE due to religious reasons, please write a note(state the reason) and bring it to the health clerk in the office.
ISPE (Independent Study Physical Education) Program: Deadline has expired for the 2024-25 school year, if you are interested in applying for ISPE next year(25-26), Interest form will be due by May 15, 2025. For additional questions or for the Intrest Form, please contact Mr. Trpin.
