Transcripts & Records Requests

Please allow up to 5 business days to process records requests.
This applies to transcripts (unofficial or official), school verification letters, attendance reports, etc.

Records Request Form Instructions:

1) Complete the entire form.

  • "Student Information" section must be filled out completely.
    • If you are unsure of the last date of attendance or graduation date, please give an estimate or time frame on when you last attended or graduated. This will help our office staff locate and process your records in a timely manner.
  • Select the "Type of Request".
    • Official Transcripts - Records are sealed in an envelope. Records become "unofficial" if opened/the seal is broken.
    • If requesting items to be mailed out, you must provide a separate, self-addressed, and stamped envelope for each item requested.
  • Select at least one "Retrieval Specification".
    • If someone else, besides yourself, will be picking up the item(s) requested, please indicate their name in the statement "I give my permission for ______ to pick up my records."
  • Print your name, sign, and date the bottom of the Records Request form.
    • Only wet signatures are accepted. NO ELECTRONIC SIGNATURES.

2) Must provide a Photo ID to authenticate your identity.

3) Payment by Cash or Money Order ONLY

  • Other forms of payment, such as checks, credit/debit card, cashless mobile apps, etc. are NOT ACCEPTED PAYMENTS for records requests.

IMPORTANT INFORMATION - PLEASE READ

  • TRANSCRIPTS: This record provides a list of classes, and credits you have earned towards a high school diploma.
  • UNOFFICIAL VS. OFFICIAL TRANSCRIPTS: An official transcript is a certified, authenticated copy of your academic record that is sealed in an envelope to ensure its integrity, typically used for purposes like college admissions or employment. An unofficial transcript is an informal copy for personal use, and is not sealed in an envelope and does not have the school's seal or signature. Please confirm the type of transcript you need, with the organization/agency who is requesting or requiring this record.
  • HISET AND GED SCORE REPORTS: Torrance Adult School (TAS) does not have access to score reports for students who took the HiSET or GED exams. Please contact the testing agency directly (HISET for HISET Score Reports; GED for GED Score Reports) to request/order score reports.
