TAS Childcare Program

• Child must be age 2-5 years old and potty trained
• Parent must be enrolled and attending a morning class at Hamilton Adult Center
• Parent must submit:
  • Child's Birth Certificate
  • Child's Immunization Records (including proof of Negative TB)
  • Three 3"x5" photos of the child
