It is MANDATORY that a copy of your child's current immunization record be provided to the FWEC (Family Welcome Enrollment Center) or directly to LAUNCH prior to their first day of school.
All paperwork listed below is required prior to starting school. Unfortunately, some of the information is duplicated so as to meet requirements by both Torrance Unified School District and the CA Department of Child Development Services for site licensing purposes.
*The TUSD Health Form, 2 completed green cards and an emergency kit are also required. However, you must obtain these forms from your child's assessing teacher or classroom teacher.
