Torrance Unified School District's Health Services has issued forms related to medication for children on the school campus.

1. Information for all parents

2. Authorization for Medication at School 

3. Authorization for Epi-pen

4. Report of Health Examination for School Entry (CHDP) DUE PRIOR TO STARTING 1st GRADE

5. Oral Health Form DUE BY MAY 31 OF FIRST YEAR OF SCHOOL (KINDERGARTEN OR FIRST GRADE)

Please note:

-Parents are allowed to excuse students from P.E. for one day. After one day, a doctor’s note is required and there must be an end date or date of the next doctor's appointment. 

-Any student who has had a fever of 101 or higher in the past 24 hours, must remain at home.  
Before students can return to school, they must be without a fever and fever reducing medication (i.e. Tylenol) for a full 24 hours.
